Most Likely Cause Of Oil Rig Fire – Open Flame Heater
Three deaths that did not need to happen are being attributed to the use of an open-flame heater which probably started the fire. The company (Dan D Drilling) apparently knew this was hazardous and had already been in violation with OSHA and was cited in June 2011.
Without basic training in place for chemical and physical hazards of chemicals on the job site along with not providing safety standards in the use of flame resistant clothing has contributed to the problem.
